Healing Session Disclaimer, Informed Consent and Release of Liability Form
Sufi Spiritual Healing uses the ancient teachings of healing and spirituality in the Prophetic traditions. We will work with the light and energy of the One Highest Divine Source, which exists for all people no matter what religion. Everything comes from this One Divine Source that heals, teaches us and transforms us. All healing comes from God, the source of everything.

We will work with prayer, guidance, and the light/energy of the names of the attributes of the One Highest Source.

Our work together is to help you discover, understand and purify/transform the inner beliefs, emotions, thoughts, and experiences that may be tied to an underlying disease, as well as emotional, spiritual, or mental level “suffering” that causes blockages in our life.
Sufi Spiritual Healing works to uncover the hidden spiritual, psychological, emotional,
and energetic aspects of mind, body, and spirit as a complement to conventional medicine treatments. In cases of diseases, this approach integrates spiritual healing with conventional western medicine, providing a synchronistic balance to help increase the possibility of complete and holistic healing. Complementary medicines, including homeopathy, Chinese medicine, and other traditions also work synergistically with Spiritual Healing.

Spiritual healing is not a substitute for a medical examination or treatment. Any advice given by Sara Elijah M.Div.is not intended to take the place of or be a substitute for standard professional medical care, examinations, and treatment. All clients are expected to visit their primary care physicians, receive medical check-ups, and follow the advice and instructions of your licensed medical professional while working in conjunction with me. Healings are complimentary to western medicine care. If you choose not to follow the advice of your medical professional, please understand that this choice is at your discretion and not my advice. I ask that you please notify me if you make this choice, so that I am aware.

You agree that you have revealed or will reveal to Sara Elijah M.Div. any and all illnesses you are aware of and that I may need to be informed of to support your healing process. You also agree to keep Sara Elijah M.Div. updated on your mental, emotional, and physical health, while you are my client.

No guarantees have been made that spiritual healing will heal your disease, or that it will disappear, remit, or transform for you.

You understand that Sara Elijah M.Div. may offer her opinion concerning your spiritual health and how it may relate to your mental, emotional, and physical well-being. This is done through intuitive and Divine inspiration and is not a hard science. What is seen and interpreted may change through increased awareness and application of these healing techniques. I honor your right to receive and use the advice of Sara Elijah M.Div. in the manner you see fit for yourself. You also acknowledge that there is no guarantee of outcome in acting upon said advice.

You acknowledge that Sara Elijah is an intuitive spiritual healer and that she offers spiritual healings for the purpose of providing mental, emotional, physical, and spiritual support using techniques learned, as well as intuitive healing methods.

You recognize that Sara Elijah may suggest or advise medical or therapeutic advice and it is my responsibility to consider such advisement as being for my benefit and not for my harm. Rabia Sara Elijah is not a medical doctor and is not licensed to diagnose illness, disease, or mental disorders, nor can she prescribe medical treatment or pharmaceuticals.
If at any time you feel endangered by what is brought up during, or after your healing session(s), I advise that you seek medical and/or psychiatric care from a physician or other qualified healthcare provider and you give Sara Elijah M.Div. permission to reach out to your licensed medical professional(s) if necessary. In an emergency, please call 911 or call for emergency medical help on the nearest telephone.

Confidentiality
All client information and records are treated in a confidential manner and no information will be released to anyone without your prior written consent, except in situations governed by law.

Session Information
Remote energy services involve the use of secure interactive videoconferencing devices and equipment that enable the practitioner to deliver services to the client virtually when they are located at different sites.

I understand that the same standard of care applies to a remote session as applies to an in-person visit.

I understand that the laws that protect the privacy and confidentiality of personal information apply to remote sessions.

I understand that I will not be physically in the same room as the practitioner. Consent will be obtained if anyone other than the practitioner and client are to be present.

Risks to Confidentiality: I understand that as remote sessions take place outside of the practitioner’s office, there is potential for other people to overhear conversations if I am not in a private place during the session. The practitioner will take reasonable steps to ensure privacy. It is my responsibility to find a private place for the session where I will not be interrupted. I also understand that it is my responsibility to protect the privacy of our session on the device being used.

Technology Issues: I understand that there are potential risks to using technology, including service interruptions, interception, and technical difficulties. For example, technology may stop working during a session, other people might be able to get access to our private conversations, or stored data could be accessed by unauthorized people or companies. If it is determined that the video conferencing equipment or connection is not adequate, I understand that either the practitioner or I may discontinue the remote session and make other arrangements to complete the session.

I understand that I have the right to refuse to participate or decide to stop participating in a remote session, and that my refusal will be documented in my client record. I also understand that my refusal will not affect my right to future care or treatment.

I may revoke my consent at any time by contacting my energy practitioner

I understand that my practitioner will maintain a record of our session in the same way they maintain records of in-person sessions in accordance with their policies.

I understand that this document will become a part of my personal record.

Hold Harmless Clause
Except in the case of gross negligence or malpractice, I or my representative(s) agree to fully release and hold harmless, Sara Elijah M.Div. from and against any and all claims or liability of whatsoever kind or nature arising out of, or in connection, with my session(s).

You attest that you understand the nature of the healing session(s) and freely elect to receive the healing(s). You also understand that spiritual healing and growth is ultimately given to you from God and is not a guarantee. Sara Elijah M.Div. will attempt to support you in ways that accelerate your healing process.

I hereby acknowledge that I have voluntarily requested a paid or unpaid individualized healing session, class, or workshop, with Sara Elijah M.Div. and consent to care and services to include, but not be limited to ailment consultation, spiritual or energetic healing, intuitive coaching, and intuitive guidance.

In consideration of this agreement with Sara Elijah M.Div. I do here and forever release and discharge, and hereby hold harmless, Sara Elijah M.Div. and all respective agents, heirs, assigns, from any and all claims, demands, damages, rights of action, or causes of action, present or future, arising out of or connected with my participation in a session or event, and as a result of following advice tendered. By signing this document, I am waiving any right I, or my successors, might have to bring any legal action or assert a claim against Sara Elijah M.Div. for accusations of malpractice, negligence, non-disclosure, or lack of informed consent. This agreement supersedes all prior and contemporaneous understandings, agreements, representations, both written and oral, with respect to such subject matter.

I acknowledge and agree that I assume the risks associated with all offerings, services, classes, and/or programs in which I participate. I acknowledge and agree that no warranties have been made to me as to the results I will achieve from any insight, guidance, healing session or program.

I understand that results are individual and may vary. I have been informed that client information and records will be handled professionally and kept confidential unless permission has been given by you to share your information for consultation purposes.

I acknowledge that I have thoroughly read and understand this agreement, waiver, and release, I freely accept all its terms, and fully understand that it is a release of liability.
